Prinsip-Prinsip Komunikasi dalam Perawatan Paliatif dan Akhir Kehidupan

Abstract

Communication is one of the determining factors in improving the quality of the care process. Communication principles are very necessary in palliative patient care. This review aimed to determine the principles of communication in palliative and end-of-life nursing. This study was a systematic review using articles from two databases, namely PubMed and Science Direct. The search keywords used were palliative care OR End-of-Life Care AND communication. With systematic screening and eligibility assessment, 8 articles were finally obtained for review. The review found the main themes, namely the principles of internal communication, namely knowledge, coaching, education and training, roles, collaboration between health workers; and the principles of external communication, namely establishing a therapeutic relationship with the patient or their relatives, empathy, involving or collaborating with the patient or their relatives, and challenges in communication, namely time, communication techniques, condition or characteristics of the patient or relatives, use of terms, and long distance communication. Furthermore, it was concluded that the care process for palliative patients was carried out by applying communication principles. What the care team needs to prepare is to fulfill internal principles first before applying external principles, as well as considering several challenges in communication.

ABSTRAK

Komunikasi menjadi salah satu faktor penentu dalam meningkatkan kualitas proses perawatan. Prinsip-prinsip komunikasi sangat diperlukan dalam perawatan pasien paliatif. Tinjuan ini bertujuan untuk mengetahui prinsip-prinsip komunikasi dalam keperawatan paliatif dan akhir hidup hayat. Studi ini merupakan systematic review dengan menggunakan artikel dari dua database yaitu PubMed dan Science Direct. Kata kunci pencarian yang digunakan adalah palliative care OR End-of-Life Care AND communication. Dengan penyaringan dan penilaian kelayakan yang sistematis, akhirnya didapatkan 8 artikel untuk ditinjau. Tinjauan menemukan tema utama yaitu prinsip internal communication yaitu pengetahuan, pembinaan, pendidikan dan pelatihan, peran, kolaborasi antara tenaga kesehatan; dan prinsip external communication yaitu menjalin hubungan terapeutik dengan pasien atau kerabatnya, empati, melibatkan atau berkolaborasi dengan pasien atau kerabatnya, dan tantangan dalam komunikasi yaitu waktu, teknik komunikasi, kondisi atau karakteristik pasien atau kerabatnya, penggunaan istilah, serta komunikasi jarak jauh. Selanjutnya disimpulkan bahwa proses perawatan pada pasien paliatif dilakukan dengan menerapkan prinsip-prinsip komunikasi. Hal yang perlu disiapkan oleh tim perawatan adalah dengan memenuhi prinsip internal lebih awal sebelum menerapkan prinsip external, serta mempertimbangan beberapa tantangan dalam komunikasi.
