Rendahnya Pelaporan Insiden Keselamatan Pasien (IKP) Menghambat Perbaikan Mutu di Rumah Sakit RSPG Cisarua Bogor

Abstract


Monitoring of patient safety incidents is carried out through analysis of reports submitted to the hospital quality committee team. This study aimed to describe the incidence of patient safety incidents based on 6 patient safety targets. This type of research was descriptive quantitative with a cross-sectional approach. The research location was Dr. M. Goenawan Partowidigdo (RSPG) Cisarua, Bogor, West Java. The variables studied were incidents reported in that time period consisting of sentinel-related incidents, unexpected events, non-injury events, near-injury events, and potential injury events. Data was collected through documentation studies based on patient safety incident report data linked to Agency for Healthcare Research and Quality (AHRQ) questionnaire results data related to patient safety. Data were analyzed descriptively. The results of the study showed that a total of 25 incidents were reported with 0 sentinels, 5 unexpected incidents, 4 non-injury incidents, 4 near-injury incidents, and 12 potential injury incidents. It was concluded that not all of the incident figures were reported to the hospital quality committee so that incidents continued to occur repeatedly resulting in no evaluation and analysis and no reporting.

ABSTRAK

 

Monitoring insiden keselamatan pasien dilakukan melalui analisis laporan yang terlapor ke dalam tim komite mutu rumah sakit. Penelitian ini bertujuan untuk mendeskripsikan angka kejadian insiden keselamatan pasien berdasarkan 6 sasaran keselamatan pasien. Jenis penelitian ini adalah deskriptif kuantitatif dengan pendekatan cross-sectional. Lokasi penelitian adalah Rumah Sakit dr. M. Goenawan Partowidigdo (RSPG) Cisarua, Bogor, Jawa Barat. Variabel yang diteliti ialah insiden yang dilaporkan pada rentang waktu tersebut terdiri dari insiden terkait sentinel, kejadian tidak diharapkan, kejadian tidak cidera, kejadian nyaris cidera, dan kejadian potensial cidera. Data dikumpulkan melalui studi dokumentasi berdasarkan data laporan insiden keselamatan pasien dihubungkan dengan data hasil kuesioner Agency for Healthcare Research and Quality (AHRQ) yang berhubungan dengan keselamatan pasien. Data dianalisis secara deskriptif. Hasil penelitian menunjukkan bahwa insiden yang terlaporkan dengan total 25 insiden dengan 0 sentinel, 5 kejadian tidak diharapkan, 4 kejadian tidak cidera, 4 kejadian nyaris cidera, dan 12 kejadian potensial cidera. Disimpulkan bahwa angka insiden tidak seluruh laporan terlapor ke dalam komite mutu rumah sakit sehingga masih terus berulang insiden yang terjadi sehingga tidak adanya evaluasi dan analisis dengan tidak adanya pelaporan.
