Hubungan Antara Arkus Pedis dengan Keseimbangan, Q-Angle dan Fasitis Plantar

Abstract

Disorders of the musculoskeletal system in the feet most often cause health problems in the elderly due to a decrease in the anatomical and functional structures of the body. Arkus spreads the load on the pedis, working not only as a shock absorber, but also as a support during activities. Disorders in the elderly are associated with pain and postural imbalance. Structural abnormalities of the arch of the pedis cause abnormalities in the quadriceps angle so that it can affect the Q-Angle value which will result in immobility and instability. In addition, structural abnormalities of the arch of the pedis will cause abnormalities in the plantar fascia so that it can cause pain, thus disturbing the elderly when doing activities. This study aims to analyze the relationship between the arch of the pedis with balance, Q-Angle and plantar fasciitis. This study applied a cross-sectional design. The independent variable was the assessment of the arch of the pedis using the Staheli method, while the dependent variable was the Q-angle, balance and plantar fasciitis. The research sample was 24 elderly people in Panti Wredha Pucang Gading, Semarang, which were selected using a total sampling technique, which involved all the elderly who met the inclusion criteria. The collected data were analyzed using Spearman's correlation test for Q-Angle and plantar fasciitis; and Fisher's exact test for balance. The results of the analysis for the right Q-Angle variable was p = 0.000 and r = -0.743; for the left Q-Angle was p = 0.000 and r = -0.776); for plantar fasciitis of the right foot, p = 0.032 and r = 0.438); for plantar fasciitis of the left foot, p = 0.020 and r = 0.473); for the right balance was p = 0.076; and for the left balance p = 0.022; r = 0.559. Furthermore, it can be concluded that the arch of the pedis is related to the Q-Angle, plantar fasciitis and balance.

 ABSTRAK

Gangguan sistem muskuloskeletal pada kaki paling sering menyebabkan masalah kesehatan pada lansia karena adanya penurunan struktur anatomi dan fungsional tubuh. Arkus menyebarkan beban pada pedis, bekerja tidak hanya sebagai penyerap benturan, tetapi juga sebagai tumpuan selama beraktifitas. Gangguan pada lansia dikaitkan dengan nyeri dan ketidakseimbangan postural. Kelainan struktur arkus pedis menyebabkan kelainan pada sudut quadriceps sehingga dapat mempengaruhi nilai Q-Angle yang akan berakibat immobility dan instability. Selain itu, kelainan struktur arkus pedis akan menyebabkan kelainan pada fasia plantaris sehingga dapat menyebabkan rasa nyeri, sehingga mengganggu lansia saat melakukan aktifitas. Penelitian ini bertujuan untuk menganalisis hubungan antara arkus pedis dengan keseimbangan, Q-Angle dan fasitis plantar. Penelitian ini menerapkan rancangan cross-sectional. Variabel bebas adalah penilaian arkus pedis menggunakan metode Staheli, sedangkan variabel terikat adalah Q-angle, keseimbangan dan fasitis plantar. Sampel penelitian adalah 24 lansia di Panti Wredha Pucang Gading, Semarang, yang dipilih dengan teknik total sampling, yang melibatkan semua lansia yang memenuhi kriteria inklusi. Data yang telah dikumpulkan dianalisis menggunakan uji korelasi Spearman untuk Q-Angle dan fasitis plantar; dan Fisher’s exact test untuk keseimbangan. Hasil analisis untuk variabel Q-Angle kanan adalah p = 0,000 dan  r = -0,743; untuk Q-Angle kiri adalah p = 0,000 dan r = -0,776); untuk fasitis plantaris kaki kanan adalah p = 0,032 dan r = 0,438); untuk fasitis plantaris kaki kiri adalah p = 0,020 dan r = 0,473); untuk keseimbangan kanan adalah p = 0,076; dan untuk keseimbangan kiri p = 0,022; r = 0,559. Selanjutnya dapat disimpulkan bahwa arkus pedis berhubungan dengan Q-Angle, fasitis plantar dan kesimbangan.
