Optimalisasi Penekanan Sinyal Lemak pada Regio Lidah dan Kompatibilitas Kontras Gadolinium dalam Protokol Pemeriksaan Magnetic Resonance Imaging pada Lidah
Abstract
Squamous cell carcinoma of the tongue is one of the most prevalent oral cancers, particularly in the anterior two-thirds of the tongue. Accurate imaging is crucial in establishing the diagnosis and determining the lesion boundaries. Magnetic resonance imaging of the tongue using contrast media can be used to confirm the diagnosis of squamous cell carcinoma. The spectral presaturation inversion recovery technique in magnetic resonance imaging can be used as an effective fat suppression method to display detailed images of the pathology and anatomy of the oral cavity and its structures, including the tongue and oropharynx. This study was conducted at Sanjiwani Regional General Hospital, Gianyar, using 1.5 Tesla magnetic resonance imaging. This involved three patients diagnosed with squamous cell carcinoma, two radiologists, and one radiographer. The examination was performed using the spectral presaturation inversion recovery method in the pre- and post-contrast phases. The image results demonstrated the effectiveness of spectral presaturation inversion recovery for fat suppression with optimal tissue contrast and the ability to display infiltration into the surrounding tissue well. Based on the research results, it was concluded that the spectral presaturation inversion recovery method is an effective technique for assessing squamous cell carcinoma due to its excellent fat suppression and compatibility with gadolinium contrast, thus assisting radiologists in determining lesion boundaries.

ABSTRAKKarsinoma sel skuamosa pada lidah merupakan salah satu kanker rongga mulut dengan prevalensi tinggi, terutama di bagian dua pertiga anterior lidah. Evaluasi pencitraan yang akurat sangat penting dalam menegakkan diagnosis dan menentukan batas lesi. Magnetic Resonance Imaging lidah dengan menggunakan kontras media dapat digunakan untuk menegakkan diagnosis karsinoma sel skuamosa. Teknik spectral presaturation inversion recovery pada magnetic resonance imaging dapat digunakan sebagai metode supresi lemak yang efektif untuk menampilkan gambaran detail patologi dan anatomi rongga mulut serta struktur di dalamnya termasuk lidah dan orofaring. Studi ini dilakukan di Rumah Sakit Umum Daerah Sanjiwani Gianyar menggunakan magnetic resonance imaging 1,5 Tesla, yang melibatkan 3 pasien dengan diagnosis karsinoma sel skuamosa, 2 dokter spesialis radiologi, dan 1 radiografer. Pemeriksaan dilakukan menggunakan metode spectral presaturation inversion recovery pada fase pra dan pasca pemberian kontras. Hasil citra menunjukkan efektivitas spectral presaturation inversion recovery untuk supresi lemak dengan kontras jaringan yang optimal serta mampu menampilkan infiltrasi ke jaringan sekitarnya dengan baik. Berdasarkan hasil penelitian, disimpulkan bahwa metode spectral presaturation inversion recovery merupakan teknik yang efektif untuk menilai karsinoma sel skuamosa karena mampu mensupresi lemak dengan baik serta kompatibel dengan penggunaan kontras gadolinium sehingga membantu dokter spesialis radiologi dalam menentukan batas lesi.
