Kebutuhan Klien dan Caregiver, Kualitas Perawatan, Desain Program dan Peran Tenaga Kesehatan sebagai Penentu Keberhasilan Penerapan Total Care di Komunitas

Abstract


Home or community health care is increasingly popular, driven by increasing life expectancy and the rise in chronic diseases. Total care is a multidisciplinary approach that encompasses physical, psychological, social, and spiritual aspects. The success of total care implementation depends on meeting the needs of clients and their satisfaction with the services. The purpose of this study was to determine the factors that influence client and family satisfaction in the implementation of total care in the community. This systematic review followed the PRISMA guidelines, with a literature search in ProQuest, ScienceDirect, Scopus, and PubMed. The keywords used were "total care patient," "home care" and "patient satisfaction." Inclusion criteria included clients in palliative care and chronic diseases, with studies conducted in the last 10 years. The results of the analysis showed that the quality of care was influenced by client needs, satisfaction, the role of caregivers, and health workers. Good interactions between clients, families, and the health team increased satisfaction. However, challenges such as caregiver burden, ineffective communication, and limited access to resources still exist. Furthermore, it was concluded that the success of total care in the community requires a holistic and collaborative approach. Understanding the challenges faced by caregivers and improving communication and access to resources are key to improving client and family satisfaction in home-based health care.

ABSTRAK

 

Pelayanan kesehatan di rumah atau komunitas semakin populer, didorong oleh peningkatan harapan hidup dan peningkatan penyakit kronis. Total care adalah pendekatan multidisiplin yang mencakup aspek fisik, psikologis, sosial, dan spiritual. Keberhasilan penerapan total care bergantung pada pemenuhan kebutuhan klien dan kepuasan mereka terhadap layanan. Tujuan dari studi ini adalah mengetahui faktor-faktor yang mempengaruhi kepuasan klien dan keluarga dalam penerapan total care di masyarakat. Tinjauan sistematis ini mengikuti pedoman PRISMA, dengan pencarian literatur di ProQuest, ScienceDirect, Scopus, dan PubMed. Kata kunci yang digunakan adalah "total care patient," "home care" dan "patient satisfaction." Kriteria inklusi meliputi klien dalam perawatan paliatif dan penyakit kronis, dengan penelitian yang dilakukan dalam 10 tahun terakhir. Hasil analisis menunjukkan bahwa kualitas layanan dipengaruhi oleh kebutuhan klien, kepuasan, peran pengasuh, dan tenaga kesehatan. Interaksi yang baik antara klien, keluarga, dan tim kesehatan meningkatkan kepuasan. Namun, tantangan seperti beban pengasuh, komunikasi yang kurang efektif, dan akses terbatas terhadap sumber daya masih ada. Selanjutnya disimpulkan bahwa keberhasilan total care di komunitas memerlukan pendekatan holistik dan kolaboratif. Memahami tantangan yang dihadapi pengasuh dan meningkatkan komunikasi serta akses terhadap sumber daya adalah kunci untuk meningkatkan kepuasan klien dan keluarga dalam pelayanan kesehatan berbasis rumah.
