Manajemen Strategik dalam Antisipasi Klaim Pending Badan Penyelenggara Jaminan Sosial di Rumah Sakit Pemerintah

Abstract


Pending claims can disrupt cash flow, increase administrative burdens, and reduce the quality of healthcare services. Strategic efforts are needed to ensure hospitals can sustainably anticipate these issues. The objective of this study was to identify the factors causing pending BPJS claims and analyze the management strategies implemented by government hospitals through a systematic literature review. This study was a protocol-based systematic review. The literature search was conducted through PubMed, Google Scholar, and Garuda Indonesia from 2020–2025. Of the 623 articles found, 10 met the inclusion criteria and were subsequently subjected to thematic synthesis. The study results indicate that the number of pending claims in government hospitals ranges from 8–30%. The main factors causing pending claims include administrative aspects (incomplete documents), clinical (inappropriate medical resumes), coding errors, and limitations in hospital information systems. Reported strategies include optimizing planning, organizing, actuating, and controlling, digitizing the claims system, improving human resource competency, routine monitoring, and collaboration with financing providers. The analysis shows that a strategy that integrates internal factors (human resources, standard operating procedures, and technology) with external factors (financing agency regulations) has the most significant impact on reducing pending claims. In conclusion, pending claims are a systemic problem predominantly driven by internal hospital factors. The implementation of strategic management, combining digitalization, human resource development, and external collaboration, has proven to be the most effective approach.

ABSTRAK

 

Klaim pending dapat mengganggu arus kas, meningkatkan beban administratif, serta menurunkan mutu pelayanan kesehatan. Upaya strategis diperlukan agar rumah sakit mampu mengantisipasi masalah ini secara berkelanjutan. Tujuan studi ini adalah mengidentifikasi faktor penyebab klaim pending BPJS dan menganalisis strategi manajemen yang diterapkan rumah sakit pemerintah melalui tinjauan sistematis literatur. Studi ini merupakan systematic review berbasis protokol. Pencarian literatur dilakukan melalui PubMed, Google Scholar dan Garuda sejak tahun 2020–2025. Dari 623 artikel yang ditemukan, sebanyak 10 artikel memenuhi kriteria inklusi dan selanjutnya dilakukan sintesis tematik. Hasil studi menunjukkan bahwa angka klaim pending di rumah sakit pemerintah berkisar antara 8–30%. Faktor utama penyebab pending meliputi aspek administratif (dokumen tidak lengkap), klinis (resume medis tidak sesuai), kesalahan pengkodean, serta keterbatasan sistem informasi rumah sakit. Strategi yang dilaporkan mencakup optimalisasi planning, organizing, actuating and controlling, digitalisasi sistem klaim, peningkatan kompetensi sumberdaya manusia, pemantauan rutin, serta kolaborasi dengan penyelenggara pembiayaan. Analisis menunjukkan bahwa strategi yang mengintegrasikan faktor internal (sumberdaya manusia, standard operating procedures dan teknologi) dengan faktor eksternal (regulasi badan penyelenggara pembiayaan) memberikan dampak paling signifikan dalam menekan angka klaim pending. Sebagai kesimpulan, klaim pending merupakan masalah sistemik yang dominan dipicu oleh faktor internal rumah sakit. Penerapan manajemen strategik dengan kombinasi digitalisasi, penguatan sumberdaya manusia dan kolaborasi eksternal terbukti menjadi pendekatan paling efektif.
